Sign and Submit on behalf of a Prescriber

If you have been delegated by a Prescriber to be able to sign Enrolment Forms, digital Prior Authorization Forms, or Documents on their behalf you will be able to sign and submit through Auxita.

The Healthcare Provider must first submit an official request to add a delegate. This is done by sending a medical directive to Auxita for authorization at help@auxita.com

Pencil iconNOTE If there are multiple Prescribers in your Facility, make sure the Prescriber is selected in the dropdown for whom you are authorized to sign on behalf of, or you will not be able to sign.

Use existing workflows and processes to initiate an Enrolment or digital Prior Authorization for a patient.  

Once you have been granted delegated authority, you will have the ability to complete the Prescriber's Consent.

  1. Check the View as Delegate checkbox in the top left corner of the form.
  2. Click Apply to add your saved signature, or manually draw it in.

    1. It will be noted under the signature box next to Reason: Signing as Delegated Authority.

  3. Once you have signed the Document or Form as a delegate, click Submit to send it to the Program, provided all required information is complete.

Pencil iconNOTE By clicking the Preview button on the form, you’ll see a note indicating the delegate signed on behalf of the Prescriber where the Physician's Signature Box is located.  A note indicating the delegate has signed on behalf of the Prescriber is shown next to the signature.
