Sign and Submit on behalf of a Prescriber

If you have been delegated by a Prescriber to be able to sign Enrolment Forms, digital Prior Authorization Forms, or documents on their behalf you will now be able to sign and submit through Auxita.

First, a Healthcare Provider must make an official request to add you as their delegate. This is done by submitting a medical directive to Auxita to authorization. To inquire further, please reach out to help@auxita.com   

Use existing workflows and processes to initiate an Enrolment or digital Prior Authorization for a patient.

If you have been granted delegated authority, you will be able to complete the Prescriber Consent. Check the View as Delegate checkbox in the top left corner of the enrolment. It will be noted under the signature box next to Reason: Signing as Delegated Authority.

Pencil iconNOTE by clicking the Preview button on the form, you’ll see a note indicating the delegate signed on behalf of the Prescriber where the Physicians Signature Box is located.

After you have signed the document or form as a delegate, you can click Submit to send to the Patient Support Program.  
 

Pencil iconNOTE If there are multiple Prescribers in your Facility, make sure the Prescriber is selected in the dropdown whom you are authorized to sign on behalf of or you will not be able to sign.
