Learn how to enrol a patient in a Patient Support Program from the Patient Profile page or the Programs page.
From the Patient Profile
- Begin by selecting a patient
- Search for a Patient Support Program or select one from your Favourites.
- Open the Enrolment Form. Some fields are pre-populated with data pulled from the patient’s or prescriber details in Auxita.
- Complete the form by filling in any of the other fields.
- If a patient signature is requested on a form and the patient is in the clinic and able to sign, their signature can be captured directly on the enrolment form with the Capture Patient Consent button. How to Capture Patient Consent in Office.
- If you are logged in as a Prescriber, complete the consent and Add Signature, then press Submit.
- The form will be sent to the Patient Support Program.
From the Programs Page
- Select the Program from your list of Favourite Programs OR use the Enrolment Search bar to find the Program.
- Once you select the Program, the Enrolment Form launches.
- Complete the form by filling in the fields, as required.
- Capture Patient Consent in office, if prompted on the form.
- If you are logged in as a Health Care Provider, complete the consent and Add Signature, then press Submit.
NOTE The Enrolment must be signed by the prescribing physician before you can Submit to the program.
Signatures cannot be saved for later submission. If you click Save and Close after signing, the enrolment will be saved as a Draft and you will need to sign it again when you reopen the file.
Only users with prescribing access are able to Submit the final enrolment.
