How to Complete a Consent or Insurance Request

Healthcare Providers and Patient Support Programs work with Auxita for your specialty treatment. Occasionally you may receive notifications from Auxita asking you to complete an action like providing consent or insurance information.

Your consent is required to complete your enrolment into a Patient Support Program for your specialty treatment. 

Healthcare Providers can collect this consent with a physical signature in office or send you an email through the Auxita platform to provide your consent digitally.

You will be prompted to confirm your identity by entering in your date of birth.  

A patient will receive a prompt asking them to confirm their identity by adding their date of birth

Click Confirm This is Me to continue.  

Next, you may be prompted to Provide Consent.

Each consent document may look different, depending on the Program or type of consent. Read through the form displayed on the screen, complete any required fields and electronically sign your name.

Click the pink button to continue.

Lastly, if your Healthcare Provider does not already have your insurance details saved, you may be prompted to provide those details.  

Select the Insurer Name from the list. If your insurer is not listed, or you do not have private coverage, select the Province you reside in.  

Fill in the rest of the details including Plan Contract Number, Member Certificate Number and Sponsor.  

If you have more than one insurance plan, click Add Another Plan and fill in the details as required.

Then press Submit Information to complete the request.
