I Received a One-Time Use Code, what is it?

You are receiving this message because your PSP or pharmacy utilizes Auxita for streamlined processing of digital forms and documents.

Auxita is a secure digital platform designed to centralize all your PSP needs. Our e-sign feature, similar to other software you may have used in the past, offers the added advantage of automating the entire digital enrolment experience, minimizing errors and saving time for both you and your patients.

An email or fax will be sent requesting you to agree to the terms  and apply your signature to complete the digital form.

Lightbulb icon TIP Please note that one-time code links expire within 10 days from the date they were sent. If you receive a notification that the link has expired, please contact the Patient Support Program listed in the email or fax.

You may receive a reminder if you do not complete the request. Please note that once the reminder is sent, the previous link will no longer be valid. Please make sure to use the most recent link provided in the reminder email to complete your submission.

  1. Click on the link received by email, scan the QR code received via fax or directly type in the website https://my.auxita.com/guest to be redirected to the Auxita platform.
  2. You are then prompted to verify your identity by providing your one-time use code. Depending on how you accessed this page, the code may be populated for you.
  3. You must also agree to the terms and conditions before you are presented with the patient’s Prior Authorization form. Check the box I agree to the above terms.

  4. Click Proceed to continue. 
  5. Next, you are brought to the digital form that will be partially filled in by the Patient Support Program. Review the details and provide your consent by completing your electronic signature.  
  6. After completing your electronic signature, press Submit to finalize the submission of the digital form. You can always press Save & Close and return to submit the form at a later time.

  7. You will be redirected to the Thank You page, confirming that the form was successfully submitted. Should you want to subscribe or sign up for a free Auxita account, simply fill in the details on the contact form at the bottom of this page.

Pencil icon NOTE If you do not wish to proceed, you can select Decline. This notifies the Patient Support Program that sent you the One-Time Use Code so they can determine the next steps.
