Submitting an Enrolment That Does Not Require a Prescriber’s Signature

Some enrolment forms in Auxita do not require a prescriber’s signature. When submitted, these forms are for enrolment purposes only and are not considered valid prescriptions.

Pencil iconNOTE Only users with the appropriate permissions can complete and submit enrolment forms without a prescriber’s signature. If you do not have this permission, the option to submit without a signature will not be available.

Steps on how to submit the form

  1. Initiate the form from the search bar in the program section or patient's profile and begin filling it in.
  2. Once the form is filled in and ready for the next step, click Submit to continue without a signature.

    1. A warning message appears if the Physician Signature is missing. Read and acknowledge the message before proceeding.

      A warning message that states "The enrolment does not constitute a valid prescription and cannot be used as such. Continue with submission?"
  3. After confirming, the form is automatically sent to the program.

undefined-Jul-21-2023-06-20-17-8672-PM-2 NOTE Once the form has been submitted, either click the three elipses or the underlined form to open the PDF version from the status table. The downloaded PDF document displays a NO SIGNATURE watermark in the signature field if the physician has not signed.

Once the form is submitted, in the signature field on the downloaded PDF form, it will display "NO SIGNATURE" where the physicians signature would normally be seen
