How to Send Additional Documents with a Prior Authorization

Programs that support Digital Prior Authorization Forms and Document Exchange can add attachments to any Prior Authorization Form initiated through Auxita.

Steps to Send Additional Documents: 

  1. Click Attachments at the top of the Prior Authorization Form.
    A snippet of a prior authorization form displays the Attachments option selected, here you can select upload or attach from documents.  
  2. Select Upload File to select a document from your computer.
    1. The document upload window will display.
    2. The Patient and Program will automatically populate based on the current Prior Authorization. 
    3. Fill in the Document Type.
    4. The Attention field allows you to choose a recipient; however, this field is not a requirement.
    5. Click Browse to select the file on your computer to attach it. 
    6. The File Name populates based on the attachment name but can be edited. 

3.     Attach From Documents to select a previously uploaded document from the Patient’s Profile.

 a. Select the checkbox next to each document you want to send with this Prior                 Authorization form and click Attach.

4. The Attachments button indicates how many attachments have been selected.

Click the Attachments button to view which documents have been attached.

The attachments button displays a number indicating there are documents. You can click the Attachments button to see which documents are attached.

The documents that have been selected are listed under the Attachments button.

5. Upon submission to Payer, all attachments are sent with the Prior Authorization form, and any newly attached documents are saved to the Patient’s Profile.
