Select **Upload File **to select a document from your computer. 1. The document upload window will display. 2. The **Patient** and **Program** will automatically populate based on the current Prior Authorization.  3. Fill in the Document Type. 4. The **Attention** field allows you to choose a recipient; however, this field is not a requirement. 5. Click **Browse** to select the file on your computer to attach it.  6. The **File Name** populates based on the attachment name but can be edited.  3. **Attach From Documents** to select a previously uploaded document from the Patient’s Profile. 1. The **Attachments** button indicates how many attachments have been selected.
