How to Send an Enrolment to an Alternate Recipient

Prescribers and their Delegates now have the flexibility to direct Enrolment Forms to a different recipient by entering an alternative fax number.

By default, Enrolment Forms are sent directly to the Patient Support Program (PSP). However, you now have the option to choose the destination, allowing you to send an Enrolment form to a Specialty Pharmacy or another preferred location.

Steps to Change to an Alternate Recipient:

1. When a form is complete and signature has been collected, select the dropdown menu and click Alternate Recipient 

Hover mouse over Submit to Program button and it will  display a secondary option of Alternate Recipient

2. In the displayed window, enter the fax number where you want to receive the Program Enrolment form and click submit to send the fax.

After submission, the alternate recipient fax number will be displayed on the Enrolment Card, making it clear and easy to recognize that this form was not sent to the PSP (Patient Support Program). 

The patient enrolment card with a green checkmark box indicates if the enrolment was submitted. Underneath displays the submitted date and fax number it was sent to.
