Comments Sections

During any form process (such as Enrolment, Prior Authorization, or other program forms), the Comments section is used for internal communication between PSP (Patient Support Program) staff and clinic staff.

Comments are never shared with patients and are not visible on any patient-facing forms.

Additional Notes Section

The Additional Notes section is a free-text field used to include any extra details that will appear in the designated area of the form.

NOTE Not all forms will have the additional notes section; it will only be available on forms that have space for the notes to populate.  

It can be used to provide information that should be shared with the Patient Support Program or insurance payer after submission to help support the request.
