Upload supporting documentation securely through Auxita when requested to do so from a Patient Support Program.
When a Patient Support Program requires you to provide them with documentation to assist with your enrolment in a PSP or for your insurance paperwork, they can request this document from you securely through Auxita.
First, you’ll receive an email like the one below. To begin, Click Review Request.
You will be prompted to confirm your identity by entering your date of birth.
Click Confirm This is Me to continue.
Next, you will be shown the name of the Program requesting the information, the type of document they are looking for and a note about the ask.
Patient Support Programs may request documents such as:
- Insurance decision documents
- Past pharmacy medication history
- Power of Attorney documentation
Click the Browse button to find the file on your device to upload.
You can modify the File Name if desired by clicking into the field and typing a new name.
Click Submit once you have selected the correct document. And that’s it!
