Upload supporting documentation securely through Auxita when requested to do so from a Patient Support Program.
When a Patient Support Program requires documentation to assist with your enrolment in a PSP or for your insurance paperwork, they can request this document from you securely through Auxita.
First, you’ll receive an email like the one below. To begin, Click Review Request.
You will be prompted to confirm your identity by entering your date of birth.
Click Confirm This is Me to continue.
Next, the name of the Program requesting the information will be displayed, along with the type of document being requested and a note about the request.
Patient Support Programs may request documents such as:
- Insurance decision documents
- Past pharmacy medication history
- Power of Attorney documentation
Click the Browse button to find the file on your device to upload.
You can modify the File Name if desired by clicking into the field and entering a new name.
Click Submit after selecting the document.
