Checking for Patient Consent

Within a Patient’s profile under the Program Overview tab, if a consent form was completed by a patient, a button labelled Patient Consent will be visible.

How to check the consent on the PDF form

1. Select a patient who has had an enrolment started, under the Program Overview tab enrolment status will display. 
2. Click on the Patient Consent button, this will launch the PDF version of the consent form in another tab.
3. The PDF version of the Consent form can also be downloaded from here to save a copy outside Auxita.
