Select **Upload File**. The **Document Upload** window will display. 3. Click **Browse** to select one or more files from your computer, or drag and drop the files into the upload window. 4. The **File Name** for each attachment is automatically populated from the original file name. Review each attachment and update the **File Name** individually if needed. ![Document Upload dialog showing three selected files ready for upload. Click **Submit** to finish adding the attachments. 6. The **Attachments** button displays the number of documents attached to the Additional Form. Additional Form attachments must be uploaded directly from your computer. Because the Documents section is shared with the Patient Support Program and Additional Forms may exist independently of a Patient Support Program, these attachments are stored separately. ![Lightbulb icon](https://partners.auxita.com/hs-fs/hubfs/Knowledge/Lightbulb%20icon.png?width=24&height=22&name=Lightbulb%20icon.png) **TIP** If the Additional Form calls for redacted documents, ensure you redact BEFORE attaching the document to Auxita.
