Attaching Documents to Additional Forms

Users can attach supporting documents to Additional Forms when completing the digital form in Auxita.  

1. Click Attachments at the top of the Additional Form.  

2. Select Upload File to select the document from your computer and the Document Upload window will display. 

Pencil icon NOTE You cannot attach documents that are already saved on the patient’s profile to an Additional Form. You will need to attach documents directly from your computer to an Additional Form. This is because all documents stored in the patient’s Documents section are shared with the Patient Support Program. Since Additional Forms are not always connected with a Patient Support Program, these documents must behave independently. 

3. Click Browse to select the file from your computer and attach it.  
 
4. The File Name automatically populates based on the attachment name but can be edited if desired.  
 
5. Click Submit to finish adding the attachment.  
 
6. The Attachments button will indicate how many attachments have been selected and clicking the Attachments buttons will display the list of documents that are attached.

Lightbulb icon TIP If the Additional Form calls for redacted documents, ensure you redact BEFORE attaching the document to Auxita. There is not currently a way to redact documents within Auxita.
