How to Capture Patient Consent in Office

You can capture a patient’s signed consent on enrolment forms when they are in the clinic. Obtaining consent in the clinic will ensure that the program has all the information they need to begin treatment for patients as soon as possible.

First, you must initiate the Enrolment form through Auxita.
