View your submitted PSP (Patient Support Program) forms.

You can view submitted forms sent on your behalf to a rsupport program by scrolling to the Programs section in your Auxita account.  

1. Log into your Auxita account, click onto Home to view your dashboard.  
2. Click onto Programs from the tabs along the top to jump to the programs section. 
3. On the program tile for a Patient support program you’ve been enrolled in, if there are submitted forms, you can click the appropriate button to view the PDF. There may be buttons for: 

• • An Enrolment Form
  • A Reimbursement Form
  • A Patient consent Form
