Guest Prescribers: Download a PDF Copy of Your Submitted Form

After signing and submitting a form, you can download a PDF copy directly from the Thank You page for your records.

How to Download a PDF

  1. Complete and submit your form.
  2. On the Thank You page, click Download.

  3. Wait while the PDF is generated.
  4. After the PDF is generated, the download will start automatically.

  • Download links expire after 1 minute for security purposes.
  • The Download option is available for up to 24 hours after you reach the Thank You page.
  • If you leave or refresh the page, you may lose access to the download option.
